In order to compete in the 2009 Max Muscle Naturals all bodybuilding competitors must be steroid free for a period of at least one-year prior to March 28, 2009. In addition you must pass a polygraph examination at the NPC approved testing site listed below. attn: Joseph Paolella, 4311 Wilshire Blvd., Los Angeles, CA 90010. Figure competitors will not be polygraphed. Test Questions will center around: 1. Have you taken any anabolic steroids since March 29, 2007. Note: Over the counter sports enhancement products are acceptable. For examination appointment and questions call: Joseph Paolella @ (323) 965-7506. Fax (323) 965-7506 Cost of testing: $50.00, paid by athlete directly to examiner. Dates of testing: January 2nd through March 27th, 2009. Unlimited Men, Women, Figure and Bikini overall winners at the Max Muscle Naturals are not allowed to compete in this contest for a period of two years after overall win in the show. Note: It is the responsibility of each athlete to make an appointment for the polygraph examination. If you do not make it to your examination on the date and time scheduled, you will be rescheduled only if the examiner schedule permits. The event promoters are not responsible for making appointments or for payment of any fees to the examining center. The above testing center is the only authorized polygraph center. Polygraph results from any other center will not be accepted. In order to maintain a “level playing field” for competing athletes, the results are only valid for this specific event and cannot be used to qualify for any other Lindsay Productions event. The results of the examiner will be held in the strictest confidence. Individual results will be given to the event promoters and the NPC meet chairman only. Results that show concern will be discussed between the promoters and the NPC meet chairman prior to any disposition. I understand that no entry will be accepted without an entry fee. I understand that this competition is NPC sanctioned and shall be conducted in accordance with the rules and regulations established by the NPC. In consideration of your acceptance of this entry form and fee, I hereby waive and release all rights and claims for damages against the National Physique Committee of the USA, Inc., Jon Lindsay, Lindsay Productions, Veteran's Memorial Auditorium, Radisson Hotel, all sponsors and vendors, Culver City, employees or assigns by reason of injury or damage, which I incur, and include any injuries or damages suffered and sustained by members of my entourage or individuals associated with or accompanying me while traveling to and from or participating in this competition. I further agree to have my participation in this competition videotaped and photographed and I hereby waive and release all rights to said videotapes and photographs to Stolle/Lindsay Productions for their exclusive use in the illustration and/or depiction of and concerning the sport of bodybuilding. I agree to abide by all decisions of the designated officials and declare that all the information contained herein is, to the best of my knowledge, true, correct and complete. I have read and understood the contents of this entry form.

NPC CARDS CANNOT BE PURCHASED UNTIL THE DAY OF THE CONTEST, AT THE MORNING CHECK-INS.

There is a 5% postage and handling fee for ticket orders mailed.

All ticket sales are final. No refunds or transfers to other contests.

Beginning ten days out from contest date, all ticket orders will be processed and held at will call for pick-up from the auditorium box office on the day of event with photo ID.
